C & J Intake Process

Please provide your basic contact information and let us know how we can assist you today. Fields marked with an asterisk (*) are required.

Submitting this form starts your intake — it does not file a return. Filing happens only after you review and authorize your prepared return.

Agreement & electronic signature

A signed engagement agreement is part of the intake process. The agreement is reviewed with you before your return is prepared — it is not the signed tax preparation agreement itself and does not override it.
